Help Me Grow Care Coordinator
The Early Learning Coalition is seeking a compassionate, organized, and community-minded Help Me Grow Care Coordinator to serve as a trusted resource for families across our region. In this role, you will connect families with developmental, health, educational, and community supports while helping remove barriers to services and promoting healthy child development.
As a central point of contact for families, you will work closely with caregivers, healthcare providers, educators, and community organizations to ensure children and families receive the services they need to succeed.
What You'll Do
- Receive and respond to referrals from families, providers, and community partners
- Conduct intake interviews, needs assessments, and develop service plans.
- Connect families to community resources and programs that support healthy child development.
- Follow up with families by phone, virtually, or in person to ensure successful connections to services.
- Identify and address barriers that may prevent families from accessing needed supports.
- Maintain accurate records and documentation within the STAR database.
- Educate families about developmental screenings, early intervention opportunities, and available community resources.
- Support outreach events, presentations, and community engagement activities.
What We're Looking For
The ideal candidate is someone who enjoys solving problems, building relationships, and helping families navigate complex systems.
Preferred Qualifications
- Associate's or Bachelor's degree in social work, human services, education, psychology, public health, or a related field.
- Experience in care coordination, case management, family support, social services, healthcare, or a related field.
- Knowledge of child development and early childhood systems.
- Strong communication, customer service, and relationship-building skills.
- Ability to work effectively with diverse populations using a family-centered and culturally responsive approach.
- Strong organizational skills with attention to detail and documentation accuracy.
- Proficiency with Microsoft Office and database systems.
